Science as marketing

A new peer-reviewed study paints a disturbing picture of the use and abuse of science by pharmaceutical companies to help them sell drugs.


Published in the latest edition of the Journal of Bioethical Inquiry, the study goes through several case studies of big drug companies using questionable science to boost the apparent effectiveness and/or safety of their products. Authors Glen Spielmans and Peter Parry sifted through internal court documents released to the public through lawsuits against the drug companies in one of the first peer-reviewed studies to rely on such documents.


They outline several ways they say that companies used science improperly to market their drugs, including selective use of trial results (or "data fishing") to suppress or spin negative results, and ghostwriting -- in which a report written by or for a drug company to give a psitive review to one of its products is then published under the name of a respected academic or research who had little (if anything) to do with its preparation.

Questions raised about Avandia tests

Despite government investigations into the safety of diabetes drug Avandia, test subjects are still being recruited for studies into the controverial drug, according to a report.


That has some researchers scratching their heads, since regulators in Canada and the US are grappling with what to do about the drug.


"Exposing thousands of people to this drug for many years, just to see if existing studies are right, doesn't make a lot of sense," says Dr. David Juurlink, the scientist who was the lead investigator in the ICES study that compared Avandia and Actos.


"The idea that you would enroll tens of thousands of patients into a study and give them this drug for years just to see if it is really more dangerous, that to me that is something you might do to guinea pigs, not to patients," he says.

This will likely be the last blog from the Health Care Globalization Summit. Cards have been exchanged, hands shook and business deals among the delegates begun -- if not complete.


It's been fascinating for me to immerse myself into the business side of health care. It's something we don't see that often in Canada. This being a business conference, there was was no discussion of whether for-profit medical care is a good thing, or whether higher profits are a measure of quality of care -- topics that always come up when business and health care are discussed in Canada.


The question all week was not should profits be made from health care, but how can more money be made.

Steve Jobs' health privacy investigated

It seems the US Securities and Exchange Commission will soon weigh in on the tricky ethical issue of health privacy for public figures.


According to a Bloomberg report, the regulator is investigating whether investors were misled as concerns about the health of Apply CEO and founder Steve Jobs began to surface.


Investors have been pressing for information on Jobs’s health since June, when he appeared thinner at an Apple event. The company’s stock whipsawed this month after Jobs, who battled pancreatic cancer in 2004, said he would remain CEO while seeking a “relatively simple” treatment for a nutritional ailment. Nine days later, Jobs said he would take a five-month medical leave after learning his health issues were “more complex.

Speedy organ harvesting

By taking a heart from a dying infant within 75 seconds of the heart stopping, the chances of a successful transplant to another baby are greatly increased, according to a study in today's New England Journal of Medicine that is raising ethical questions about the definition of death.


The study examined the cases of three dying newborns whose hearts were taken within 75 seconds of cardiac death and given to three other infants.


"As a result of their investigational protocol, three babies are now alive; had the procedures not been performed, it is virtually certain that all six babies would be dead," the journal's executive editor Dr. Gregory Curfman and others wrote in one of several commentaries accompanying the study.

Spanish saviour sibling

Little Javier was born last week into a world of controversy.


That's because the little Spanish boy was conceived for one reason only: to provide stem cells for his older brother, Andres, who suffers from a congenital form of anaemia. Stem cells from Javier's umbilical cord will be used in a bone marrow transfer to his six-year-old sibling in hopes of curing his condition.


Javier was screened before birth to ensure he was free of the condition, which has led the Roman Catholic Church in Spain to condemn the situation, since such screenings mean other embryos that were not free of the condition were discarded.

Parents who want to pick the sex of their babies need only to travel to Turkey and pay for in-vitro fertilization there, an undercover BBC report has found.


As in Canada, sex selection is illegal in Britain is illegal, unless for medical reasons. For instance, if your family has a genetic disorder affecting only males, you can chose to have a girl.


Ironically, similar rules apply in Turkey, and yet the Jinemed Center, a fertility clinic in Istanbul, has proven a popular destination for patients wanting to pick the gender of their baby.


BBC reporter Colette McBeth took her husband and a hidden camera to a meeting with representatives from the centre and made it clear they wanted to have a girl. A video of the meeting shows the couple discussing with the representative ways to get around the law by pretending a heriditary desorder runs in their family.

Surgeon sued

The definition of informed consent for surgery may be expanded as a result of a law suit filed recently in the United States. In the future, doctors my need to tell patients if they have a financial stake in the treatment being given, beyond the usual fees for their medical services.


In fact, ethicists are saying they should disclose such information already.


The issue stems from a law suit filed by a Chicago woman who is suing a leading heart surgeon there after he implanted a device in her heart to repair a valve. The patient, 41-year-old Toni Vlahoulis, said she never would have agreed to the surgery had she known her doctor invented the device meant to keep her valve from leaking, and received royalties every time one is used.

I got an e-mail from a reader saying that she was under the impression that most doctors don't like their patients coming into their offices armed with web page printouts, convinced they already know what's wrong with them and just need a prescription written. She was responding to my last post about Health 3.0, which is web-based health care, allowing patients to largely diagnose themselves -- even take their own blood tests.

What is Lysteda?


Lysteda (tranexamic acid) is a man-made form of an amino acid (protein) called lysine. Tranexamic acid prevents enzymes in the body from breaking down blood clots.


Lysteda is used to treat heavy menstrual bleeding. Lysteda will not treat premenstrual syndrome (PMS).


The Cyklokapron brand of tranexamic acid is used to prevent bleeding in people with hemophilia who need to have a tooth pulled.

Gum Disease Linked To Higher Premature Birth Rates

The International Journal of Obstetrics and Gynaecology, BJOG, reported that a strong link has been found between successful gum disease treatment and the likelihood of giving birth prematurely.


Researchers at the University of Pennsylvania Dental that women with untreated gum disease were roughly 8 times more likely to have a premature baby than those women who received successfully treated for gum disease via scaling and root planning (cleaning above and below the gum-line). However, the study's results also suggest that pregnant women who were resistant to scaling and root planning were significantly more likely to deliver preterm babies than those where it was successful.

What is Septra?


Septra is an antibiotic combination containing sulfamethoxazole and trimethoprim and it works by killing sensitive bacteria. Both ingredients are antibiotics that treat different types of infection caused by bacteria.


Septra is used to treat ear infections, urinary tract infections, bronchitis, traveler's diarrhea, and Pneumocystis carinii pneumonia.

Milk: It Does A Tooth Good

Your mother may have told you some white lies, including that breaking a mirror is seven years bad luck or that if you keep picking your nose it would get stuck that way, but one thing that she did get right is that drinking milk will probably help your teeth out.


Studies have found that drinking milk and eating cheeses, especially cheddars, with or after a meal can help increase the pH level of your mouth (for us non-scientists out there, this means that it will lower the acidity level) thereby limiting the damage that certain foods can do to your enamel.


To avoid having to purchase the best dental coverage available just so you can afford to fix your kid's teeth later in life, make sure to include a little milk and cheese with their meals, particularly with acidic meals such as pastas with red sauce, to help protect them from cavities.

Pharmacists, dietitians to gain greater powers


Health professionals such as pharmacists, dietitians and midwives will gain greater powers under proposed legislation to be unveiled today.
Physiotherapists able to order x-rays, midwives who can put a breathing tube in the mouth of a struggling newborn, and, pharmacists able to refill prescriptions without first checking with a doctor could soon be commonplace in Ontario if the legislation passes.
Nurse practitioners will be able to order bone density tests and scans such as MRIs, set or cast a fracture or dislocation of a joint and apply specific forms of energy such as diagnostic ultrasounds.
The scope of practice changes are part of a Liberal government initiative to ease long wait times in doctors offices and hospitals and to improve patient access to healthcare, Premier Dalton McGuinty told reporters this morning.
But Ontario's nurses say the changes stop short of doing what's truly needed to ease patient flow in overcrowded hospitals by failing to give them the power to prescribe more drugs and to admit or discharge patients.
"No one professional can possibly meet all a patients needs," said McGuinty. "The next best thing is to ensure we minimize the number of stops a patient makes while travelling through the healthcare system."
Under the proposed changes, McGuinty said regulated health professionals will get enhanced power to do more for patients. Dietitians could prick skin to check a patient's blood levels for diabetes control, midwives would be able to tell patients their diagnosis, and, medical radiation technologists could, on the order of a doctor, give needles if needed.
But while the provincial government repeatedly claims they are moving to improve access to medical care, they continue to deny nurse practitioners the ability to prescribe a variety of drugs, said Doris Grinspun, the Registered Nurses' Association of Ontario's executive director. Nurse practitioners have more education and training than registered nurses. In other provinces and countries they have greater prescribing practices, said Doris Grinspun, the Registered Nurses Association of Ontario's executive direction. "Still we are needing to fight, in committee now, for open prescribing," Grinspun said, adding she hopes the proposed legislation can be amended at the committee stage before it is passed into law.
